We accept Medicare, Medicaid, and most insurance plans. Please call your private insurance for preferred provider information. Contact our clinic at 360-452-6252 to establish care with one of our providers.
To register as new patient please provide legal photo ID, insurance cards and Tribal ID/Certification of Indian Blood if applicable.
Bring all documentation and new patient paperwork to our Patient Service Representatives and we will schedule you an establish care appointment.
