Lower Elwha Health Services is here to help you with medical benefits.

Benefits Coordinators are here to advocate on the Patients behalf and help them navigate through all systems/services. They help with medical travel assistance, Medicaid/Medicare applications, and paratransit applications. Under CHS they can help with Medicare Part B reimbursements if qualified.

Medical Travel

When applying for medical travel assistance, please fill out a request and have it submitted 10 days prior to appointment for per diem.

To qualify for Medical Travel the following are required:

A copy of your-

  • VALID driver’s license
  • UP-TO-DATE vehicle registration
  • CURRENT vehicle insurance
  • Verification of appointment date and time
