Lower Elwha Health Clinic is currently accepting new patients and welcomes individuals and families from our Tribal and surrounding community. Our team provides a wide range of services to support your overall health and wellness, including medication management, naturopathic care, physicals and annual wellness visits, diabetes and nutrition education, I.V. therapy, ultrasound therapy, telehealth, psychiatric care, and pediatric services.

If you are in need of a primary care provider, we invite you to complete our new patient paperwork and return it to our front desk. Our team looks forward to partnering with you in your care and supporting your health every step of the way.

The Mental Health Program, located within the Lower Elwha Health Clinic, is dedicated to supporting the emotional and behavioral well-being of our community. Our mission is to provide culturally informed care that meets the needs of children, youth, and adults at every stage of life.

We offer support for individuals experiencing a wide range of concerns, including historical trauma, abuse, suicidal thoughts, depression, and anxiety. We also recognize that life’s challenges can affect anyone, and we are here to help those navigating temporary stress or difficult transitions.

The Lower Elwha Dental Clinic, located on the south side of the Lower Elwha Wellness Center, is committed to helping our patients achieve and maintain lifelong dental health. We believe in partnering with you on your journey to dental wellness by providing compassionate, high-quality care in a welcoming environment.

We offer a full range of services—from exams and cleanings to crowns and dentures—and proudly serve patients of all ages, including children, expectant mothers, and elders.

Dental services are available to members and first descendants of federally recognized tribes, Lower Elwha Klallam Tribe employees, and children ages 0–19 who are covered by Washington State Medicaid.

Klallam Counseling Services (KCS) is a state-approved substance use disorder treatment program offering comprehensive outpatient services. Our mission is to support both Native and non-Native individuals, as well as their families, through compassionate and culturally responsive care.

We are dedicated to helping individuals build the structure, skills, and confidence needed to make meaningful lifestyle changes. Our goal is to empower each person on their path toward healing, greater independence, and an improved quality of life.

Patient Services Representative Department

Patient Services is the first department you see when you enter the Lower Elwha Health Clinic. We deliver quality health care to members of the Lower Elwha Klallam Tribe, enrolled members of other federally recognized tribes, and most services to non-native members of the community Potential new patients need to provide a copy of the following prior to your first appointment:

  • A copy of all insurance cards
  • Social Security card copy
  • Current driver’s license or valid identification card (parents for child)
  • Copy of tribal ID or CIB
  • Birth certificate for minors or dependent care

We accept Medicare, Medicaid, and most insurance plans. Please call your private insurance for preferred provider information.

  • Maintaining/ updating patient demographics
  • Routing paperwork, phone calls and messages to the appropriate party
  • Receiving payments and co-payments
